Investigation into lapses in public hospital's treatment of pregnant woman who died

An investigation by the D.C. Department of Health found multiple shortcomings in the treatment of Somesha Ayobo, a morbidly obese pregnant woman who came to the District's only public hospital with breathing problems on June 24, 2017. Ayobo, referred to in the report as "Patient #90," died at the hospital that day. Her baby, Phoenix Somesha Carpenter, was delivered by emergency C-section and died at Children's National hospital four days later. This document is compiled from excerpts of a 127-page annual inspection of the hospital obtained by the Washington Post through a public-records request. Portions of the report that do not deal with Ayobo's case have been omitted or redacted.
